3 consecutive 2nd trimester losses

Hi Dr Braverman,
I had 3 second trimester losses and I am looking for hope from you. Below is my history.

Loss #1 @ 19 week: I had some contractions around 18 weeks. The day before the loss, i had cramping and pink discharge. I went to see the doctor, he just checked heartbeat and told me everything was fine. but next day, my water broke and i had a loss. The test result came back saying i had placenta inflamation. And my doc thought I may have IC.

Loss #2 @ 20 week: I had Mcdonald cerclage placed at 13 weeks. Again, around 18 weeks, I started to have some contractions and also i got crampings. 2 days before the loss, i went to see doctor again, because i had pink, brownish discharge and the pain around the lower abdominal. The doctor did the internal ultrasound and told me my cervix was closed and it's about 4cm, no funneling. So i shouldn't worry about it. But unfornately, 2 days later, i had really bad pain and went to ER and was told blood test showing my white blood cell count was high, which indicated i had an infection. So i had to terminate the pregnancy. The test result came back showing I was infected by E Coli. Doctor didn't know how E Coli coming into my uterus. I did bi weekly ultrasound and everytime the measurement of cervix was good round 4 cm. i also had P17 shots @ 17 weeks.

Loss #3 @ 22 week: I had Shirodkar cerclage placed at 12 weeks. My MFM did weekly ultrasound for me. My cervix length was always around 4 cm and no funneling. he also did culture test for me every 3 weeks. all results came back negative. i had P17 shots @ 18 weeks. However, started around 18 weeks, i had some contractions and bikin line pain again, which made me very nervous regarding to my prior history. so I told my concern to MFM, he thought it's just normal pregnancy symptom, i shouldn't worry because my cervix length looked good and my culture test was good. 3 days before the loss, i had beige, pinkish discharge. i got really worried, because it kinda like a copy of my prior losses. So i went to see my MFM, they did ultrasound to check my cervix, everything still looked good, no funnelling, length didn't change. The nighmare happened again 3 days later, i had really bad rectum pain and I knew I was going to labor. When I arrived the hospital, the doc can see the bulging membranes and i had 4 cm dilated. I had a loss again due to infection to E coli.

My doc thought the way i got infection may relate to my immune system. It looks like my mucous plug can't prevent the bacteria traveling from vagina to my uterus. and culture test basically means nothing to me since every test was negative. I really don't know what I can do now. and I hope you can give me some answers. Thank you!

I am so sorry for all of your losses. Getting that far and them losing them must be so diffiuclt (as all miscarriages are) .

I think that there is a chance this is not incompetant cervix as you are being treated for but may be an immune issue.

We know that women that make large amounts of anti HLA antibodies against their husband gentics can have recurrent chorioamnionitis or pre term labor. It is due to a chronic(long standing) immune attack against the placenta that evenually leads to labor and a loss.
