Dear Dr. Braverman,

I learned about you on an immunology board. Let me say how impressed I am that you take this time to help others. This is very unusual for a doctor. I'll be in NYC for a check up with my RE, and I am hoping to get a consult with you while there from out of state. Thank you in advance for assisting me.

I am a patient at AEB since March, 2011. I've had every doctor tell me to use donor egg and that I'd never conceive, but I have 11 times. All naturally, and 5 with IUI from age 39-42. My last natural pregnancy was a year ago. Each ended between 5-10 weeks. Inconclusive chromosome testing. AEB results: Borderline crossmatch at 33.8 (LAD); antiphospholipid antibody was positive; ANA was negative; Anti-ovarian was negative; Hashimoto's Disease; TSH=0.344; 50:1=20; 25:1=8.5, Cytokines 42.2; CD56=21.8; T-Reg=0.4; MTHFR A Homozygous, and AMH=.046.

I take Synthroid 75mcg, Baby Aspirin, Acyclovir 800 mg daily (the first cold sore break out ever was on my cheek after EACH IVIG treatment), two IVIG treatments in one month which followed with severe chest congestion and persistent coughing that lasted over 6 weeks; Humira; LIT, Folgard, Lovenox 30mg, Magnesium, Fish Oil, Prenatal Vitamin.

To make a long story short, after one LIT treatment in Nogales, my LAD went to 77.8 in May. After 2 Humira shots, my T Reg went to 1.2 in June; BUT AFTER 2 IVIG TREATMENTS, my 50:1=22.1, 25:1=17.2; CD56=11; Cytokines=43.8. I became very ill after IVIG with chronic cough and fatigue, and cold sores on my face (which, as mentioned, I've never had; hence, the daily meds to keep this at bay.) AEB thought maybe the cold sores were the cause of my elevated levels and the cause of no response to IVIG, but after 5 weeks of taking acyclovir, my levels have not improved.

SHOULD I USE DONOR EGG at 43 with an AMH of .046? If you say yes, I will do this and move past one obstacle. Secondly, CAN YOU HELP ME TO LOWER THESE LEVELS if I consult with you? Can INTRALIPIDS help me instead of IVIG? Not sure where to go. I'd like to do my first IVF cycle, but need to get all levels in order, AND I CANNOT TRAVEL BACK TO MEXICO FOR LIT. I'm past the end of my rope.

ok I can see you've been through quite a bit. Firslty donor egg always makes it easier at you dont have to worry about chromosomal abnormalities as the cause of your losses. It is certainly still possible to concieve with your own eggs just very low chance at age 43. If you look on my boards you can see I have several of these patients.

I do feel that all your losses are immune related if not genetic and there are things that we can do to help the cytokine levels and treat this even if you perform donor egg. Not sure for the reason for the cold sores but immunosuppression is the main reason for reactivation.

So would it be a safe bet that if you already prone to cold sores and are embarking on immune treatments you should be prepared for outbreaks?

I have also used L Lysine as a preventive and treatment in the past with some success, would this be alright to add as a supplement to daily vitamins and supplements?

Dr. Braverman,

I can't get to you soon enough. I was cleared by AEB to start my IVF cycle, but I'm feeling uneasy about things. I am on day 1 of Follistim, and I wish that I could Skype with you to review my results. You are a very busy man, rightfully so. I've found some success with ILs SLOWLY lowering my TNF and NKs that are now 35 and 20. I'm thinking I need your advise on Neupogen. I'm so overwhelmed and confused... maybe it's time to give up after 11 miscarriages. My MFM doctor changed my Lupron to Heparin, and I'm already taking Dex. I need help lowering my NKs and TNF and T-Reg. Can you PLEASSSSSSEEEEEEE help me. I'm frightened now.
