Hello Dr. Braverman,

I stopped taking birth control at age 39 (2013), after 23 years of taking it, when my husband and I decided we would see what happened without actively trying and I paid absolutely no attention to my cycle. I spontaneously became pregnant and miscarried twins in May 2016 at 9wks after spotting weeks 5 through the end of pregnancy. I had a chemical pregnancy in July 2016 followed by a miscarriage at 5 wks in August 2016.

I began seeing an RE in September 2016. RPL workup was done including Recombine Carriermap and was told all came back completely normal. I asked RE to test progesterone due to my suspicions of having a short luteal phase and was advised it is not customary due to level changes throughout the day, however in December 2016, he agreed to prescribe Endometrin 100mg/2× day 72 hrs after LH surge. Since then, I have had 2 additional chemical pregnancies; December 2016 and April 2017. I am at a loss as to where to turn. My husband and I are not open to any ART that takes the place of the marital act, therefore no IUI, IVF, donor eggs, etc. I am concerned that this decision has had an effect on how far my RE is willing to go to assist. Additionally, I am concerned that my age is influencing his decision as he himself said my RPL may simply be due to my, what he referred to as, "advanced wisdom." In addition to the losses, I experience irregular cycles (anywhere between 23 days and 28 days) including abnormal uterine bleeding (AUB) most months, even on progesterone. Thinking back, I recall having AUB even when I was on the pill, going back many years. Additionally, since coming off the pill, I have heavy, flooding periods, with large clots. There were no abnormal findings (fibroids, etc.) on 3D sonohysterogram done by RE in September 2016 yet my OBGYN saw fibroids on transvaginal ultrasound in August 2016. I am feeling very discouraged and considering discontinuing pursuing things further as I may very well be "too old" and going through perimenopause but the fact that I am still ovulating monthly and can clearly still become pregnant just won't let me give up. And I can't help but think there are things we haven't looked at. Bottom line, I am looking for your opinion as to whether or not it makes sense to continue looking into this based on my history and age. Thanks in advance.
