Dr. Braverman,

Are there testing procedures done before a patient can try G-CGH in an IVF cycle?

I read that there is a study being done here on G-CGH:

They are trying two 300iu uterine washes, separated 48 hours each. The study does not say "when" the washes occur in the cycle though. Is this done after ER and before transfer?

I think you mean C-GSF not CGH. There are several indications that we are using it for, lining issues, failed IVF cycles and recurrent losses. Depending on which one you need would determine the protocol for you. With the itnrauterine wash it is done at ER, sometimes a second does 48 hours later. There is no data yet to detemine the correct dose. If you are treating for recurrent losses you may do better with the SQ treatments, it all depends on your history.

Hi Dr. Braverman,

Thank you for the reply. I've done 9 IVF cycles (8 w/ICSI, all with AH), 1 FET, 2 IUI (before my tubes were rmm'd) without a positive b-HCG test. 29 embryos transferred in all. Each cycle I get what I think is implantation cramping for about 2 days in the middle of the 2WW then it disappears just as soon as it showed up - all bHCG tests have been < 5.0.

I've been diagnosed (by the folks at AEB) with NKs with high killing power, 4 NKUs per field (biopsy done a few years ago), elevated APAs (pretty consistent over the last 5 years), and hetero for the 1298 form of MTHFR (the milder one, yes?). I have stage 4 endo and adenomyosis (Dx by Nezhat June 2010). Cytokines have always been normal (Th1:Th2?)

I've cycled with LIT, IVIg/intralipids, 40 mg lovenox, levothyroxin, baby aspirin, fish oil, extra D3 (4000iu/day).

Embryos will survive til Day 5 (blast?) - low to zero fragmentation. Normal karyotype on my end (using DS so not sure on his end - but first 4 cycles were with a former partner whose SA was perfect). So I'm not convinced it's a sperm issue. I am an older patient (46 as of today). I've been doing the daily CCRM cocktail of 3 mg melatonin, 800mg CoQ10, and 4g Inositol since May or June. I think this might in part explain my good embryo quality. Last month I had an RI index (4 days past transfer) that ranged from 0.69 to 0.74 - on 70mg lovenox. (I weigh 174 lbs so I think 79mg/day is right for my weight?). I realize not all RIs place stock in this test - just providing data points that I can remember without my notes.

I did send you an email and received a call from you on Friday. I just flew into SF today and will try your office on Tues or Weds to see if I can come in for an appointment when I return to NYC next week. Hopefully the notes here are helpful for what I've tried thus far.

Ok Linda, I see we have a lot to discuss. Its clearly has not always been the embryos. See you when you get back to NYC.
