Sarcoid and failed 4 IVF implantations

Hello Dr. Braverman MD,

I am glad you might be able to help us and thanks for all your work you do.

I have Sarcoid, the mild form, Lofgrens. My symptoms are mild and I don't need treatment for it.

I did 3 IVFs with OE and no signs of implantation. No chemicals, nothing.

I was told it was due to old age (age span 38-41) and now I recently did DE IVF. The DE cyclyt recently failed with 2 perfect embryos. I had a hatching blast and a 4AA. No chemical, ZERO HCG!

I did have my Nkcell tested and it was high, level at 24.9. Dh at I at the time had 50% DQ alpha match.

I know that Sarcoid is Th1 dominance and (to make it short) its secreting cytokines that trigger my Nkcells to kill the embryos.

For my next fresh IVF, we are willing to do Intralipids, Lovenox, steroids, and maybe adding Neupogen. Is there additional or different treatment I should do.

I am also afraid If i "knock" out my immune response to much then I won't have the necessary immune function to have an implantation success.

Should I test for interferon gamma , Interleukin 2 and Interleukin 18 and TNF-alpha in my blood to see how much I need to suppress them. I think these are the end culprits who are not allowing me to implant.

I do want to mention the day after ET I had moderate nausea and about 4 days I broke into a heavy drenching sweat when I woke up in the middle of the night.. Are these weird symptoms that my body was over reacting to the embryos?

we need to have a consultation so I can go over all the immune testing you have had to date, then order what is missing so you can get exactly the treatment you need for what is most obvioulsy an immune issue , especially with the failure of a donor egg cycle. All the treatments you mentioned are good , but must be followed carefully and adjusted as you go through your treatments.
