Hi Dr. Braverman,

I am 31 and have been diagnosed with PCOS. In addition, further testing showed that both fallopian tubes are blocked at the uterus. I went through IVF. The first time, no mature eggs. The second time, lots of mature eggs and embryos. Nine grew into blast., two were implanted, and seven were frozen. This was done in 2009. One implanted, but at 6 weeks I miscarried. I did not have a D&C, so no genetic testing was done. However, from the beginning my numbers were not good so I had the feeling that there were some abnormalities. In December of 2010 my husband and I did a frozen embryo transfer. Again two were transferred and one implanted. Things were going great, and then without warning at 8.5 weeks I miscarried. I am devastated. On Monday of this week I had the D&C. So I hope to get some answers from the genetic testing. However, I need some additional answers. The first time around, I took metformin even after getting a positive pregnancy result. The second time around I was told to stop the metformin after the positive pregnancy result. I know metformin has some positive effects on PCOS symtpoms. I have been doing some research on the internet and came across your website and this branch of medicine. I need to investigate this avenue to determine exactly what effects are PCOS having on the embryo/fetus and if there are any immune issues that need to be treated. We have 5 frozen embryos left and I just can't experience another miscarriage. We are looking for help. Please help us.

Sincerely,

C

Hi Claretopia

I dont think stopping the metformin was responsible for your loss. While I like to continue it through pregnancy the only risk of stopping it is the development of gestational diabetes. I think your losses are more likely due to the immune or thrombophilic issues associated so often with PCOS. At this point with the history of your losses that workup must be your next step and the good news that treatments work.
If you look on our discussion board you will find patients with the same diagnosis and successful managements. Its a very common problem that we see at our practice.
Dont despair, call me and we can schedule a telephone consultation and begin your workup and management plan.
