Steroids with Neup

Hi Dr Braveran,

I'm trying to understand Neupogen a bit more.

I'm wondering -

Do you often find Neupogen rREPLACES the need for prednisolone?
(I realise it depends on individual cases, but I'm wondering whether it is more often the case).

What do you normally base your decision on in regards to adding/removing prednisolone?

Are there any health risks to taking the 2 together- would regular FBC tests be needed?

yes I usually dont add prednisone as neupogen itself can lower the inflammatory cytokines by induciton of T regulator cells. In some patients the cytokines remain elevated due to a strong immune response or due to low sensitivity of the G-CSF receptors. Prednsione has been shown to potentiate those receptors so in those cases we have added prednisone to the neupogen . We follow our immune profile very carefully during neupogen treatment cycles and patients should also have an occassional CBC to check the WBC. Only those with expereince in both should be prescribing and following these patients.
