Hi Dr Braverman,
I have one 2.5 yr old daughter, we've been trying to conceive #2 naturally for nine months, with no pregnancy achieved until positive result this am after clomid and IUI 2 weeks ago. I am terrified of reliving the IUGR/preeclampsia pregnancy I experienced with my first child. We conceived her on the 2nd month of trying naturally. I was diagnosed with pcos at 19, but on my last 2 workups with RE he said none of my results look like pcos and he wouldn't have suspected it if I hadn't mentioned it in my hx, I appeared to be ovulating fine on my own, husbands sperm analysis was perfect. I have tested positive for warm red cell antibodies and always return with a false positive rpr. Tested negative for Lupus DRVV Ratio and aPPT ratio, negative for cardiolipin IgG and IgM, negative for Beta 2 Glycoprotein 1 IgG and IgM. My current RE thinks that the positive RPR/warm red antbodies mean nothing in light of the other negatives. I am highly suspicious and my OB with my first baby agreed and said I had a good chance of going on to test positive for Lupus/RA in the future. I also have strong family history-mom had severe preeclampsia, aunt had severe preeclamsia plus has RA, grandma had a pulmonary embolism delivering my mom, mom's side of family all are hypothyroid (My TSH is 2.04). I was thinking of asking my RE for an ANA, Rheumatoid factor, and MTHFR/homocysteine test, as well as to start on baby aspirin. Sorry for the ramble, but does this seem like a reasonable/useful request.....should I do more? Thank you sooo much!

Rather than guess what you might need, your history is very significant esp with the history of toxemia with your first. You should have a complete immune w/u , HLA w/u , and fill out the thrombophilia w/u. I would be happy to assist you with this, feel free to call my office and arrange a telephone or skype consultation. Im sure i can provide you with answers and recommend the correct treatment.

Thank you for your reply:-) Pregnancy ended in "chemical pregnancy" today, so I will definitely be giving your office a call after New Years.
