Anembryonic pregnancy

Dear Dr. Braverman,

I have a complex history. 1st pregnancy conceived easily (1 cycle), ended in natural miscarriage at 6 weeks. 2nd pregnancy conceived after 2 cycles, ended in 31 week stillbirth with undiagnosed congenital heart defect. Began TTC again after several months, had multiple natural cycles unsuccessful, two IUI cycles (Clomid, Follistim) unsuccessful, fresh IVF w/ PGS unsuccessful, FET w/ 2 genetically normal embryos resulted in pregnancy. At 7w1d, diagnosed w/ anembryonic pregnancy (this was a PGS embryo). I have been on ASA and Lovenox for triple variant MTHFR and hetero Factor V Leiden. Treated w/ steroids at time of transfer Significantly elevated anti-thyroid antibodies, just started levothyroxine after TSH increased from 1.4 to 2.4.. After blighted ovum diagnosed, parental karyotypes were sent, results pending.

Do you have any concerns about an immunity issue with a genetically normal embryo that results in a blighted ovum? Could an immune issue explain secondary infertility after two successful conceptions within a period of several months?

I would be interested in any insights you may have. Thank you.

Hi Dahlia

there are many immune issues that can explain your secondary recurrrent pregnancy loss and I can work all this up for you . Most important here are to rule out either HLA matching issues between you and your husband or an inherent immune issue with you leading to an overactive immune system during implantatation and during your first trimester.
