Chronic Histiocytic Intervillositis

Hello Dr Braverman,

I have had 7 miscarriages over the last couple of years all ranging from 6wks-14wks. I have been diagnosed with Hetero Prothombin Gene mutation and Homo MTHFR C copies. The placental pathology report for my latest miscarriage came back with a diagnosis of massive chronic histiocytic intervillositis (CHI). I am already being treated with daily low dose aspirin, folgard, prenatal vitamin and upon a postive pregnancy test I start progesterone supplements and lovenox.

From my understanding CHI is an immune disorder attacking the placenta and has a high recurrence rate. My question is how common is CHI and what is a treatment protocol for this diagnosis?

Many immune issues can result in CHI so that is just a symptom. You need a full immune evalutation , Im not sure that what you are on now will do. Please call me for a brief discussion and I can fill you in on my thoughts, but I think I need to give you a complete immune workup .

I have had 4 miscarriages by now within 2 years. We don't know the cause of the first two miscarriages. In the third miscarriage, placenta was tested and I was diagnosed with Chronic intervillositis. I am going to have D & C on Monday on my 4th miscarriage. I was on Lovenox (30mg/0.3 ml) and baby aspirin after 5 weeks of pregnancy and it did not work on the 4th pregnancy. I was also on 2 pills (800mcg folic acid). I am also heterozygous for the MTHFR A1298C variant(one copy). The MTHFR C677T variant was not identified

(1) Did these combination of medicine worked for you?
(2) Dr. Beaverman, have you tested cases with Chronic intervillositis with success? If so, how many cases? what worked?
(3) Have you had any cases with my set of conditions?

our understanding of CHI is that it is most likely an antibody mediated issue and you should have full testing for anti paternal HLA antibodies. Treatments from IVIG to even Plasmaphoresis are still mostly theoretical but there is excellent support in the literature that this should be effective. We have seen several cases here as the diagnosis is gaining acceptance as a treatable autoimmune condition but the numbers treated are still low.
