New literature has shown that when both TNF(tumor necrosis factor) and INFgamma(interferon gamma) are elevated there is a combined increased effect of trophoblast cell apoptosis(destruction of the cells of the invading embryo). We always test for these levels in my practice. This is why patients with very elevated levels early in the pregnancy have higher rates of pregnancy loss and if the pregnancy makes it past the first trimester, the placental damage that is done leads to higher rates of third trimester problems that are associated with placental (trophoblast) destruction. These are toxemia , intrauterine growth retardation, Maternal Liver disease and even still birth. Treating these issues early on has been a focus of our treatment for our patients and has been shown to reduce the miscarriage rates as well as third trimester problems.

COuld you please elaborate on how you treat these? Also, what if they become elevated with IVIg during the first 5-6 weeks of pregnancy? How do you treat them then?

These are very old posts and our knowledge and abiltiy to be more specific with our testing allows us to look at many other factors along with cytokine elevations, including serum cytokines and not just intra cellular cytokine ratios. But typically if IVIG is not working we look at adjusting the dose and or increasing prednisone. If its prep prior to pregnancy we may use drugs such as Humira and we are exploring other new cytokine antibody preperations.
