Endometrial function test

Dr Braverman,

I have consulted with you. Have done 3 transfers. 2 BFNs, last one chemical pregnancy with neupogen. What do you think of the endometrial function test that is done by Yale university? Do you think this test is helpful and should I do the test? It seems the test tells you how much progesterone to take.I have adenomyosis on the endometrial lining so I know I have issues so is it still necessary to take the test. Also can neupogen alone treat adenomyosis? You recommended letrozole but I have done lots of research that depot lupron is the main treatment for adenomyosis/endo.

the literature on letrazole is new , lupron has been used for a long time with the main problem being length of treatment and symptoms from treatment. The literature is also conflicting on the cuase of implantation failure with adenomyosis. Neupogen is too new to determine its benefit with adenomyosis, it most likely will not affect the aromatase enzyme that letrazole helps to deactivate. But will help with some of the immunologic problems that are inherent with endo/adeno myosis.
Im not against a long course of lupron if all esle fails, but I have rarely had to resort to that which tells me it is most likely not necessary. It however does not mean that a patient who has failed all other therapy would not benefit from treatment, but as I said , its just a lot to go through.
