Immune Issue?

Hi Dr. Braverman,

I have been trying to conceive for 4 years. The first two years I had unsuccessful IUI's and an IVF using my own eggs. Finally after two years my RE indicated my egg quality was not good, I had elevated FSH (18), I Was 36 when I started. At the IVF, there were 5 mature eggs retrieved but only one fertilized. I moved to an egg donor.

1st egg donor only produced one embryo and the cycle was not a success,
I was told this time it was bad luck on the egg donor that I used.

2nd egg donor cycle, 7 mature eggs that fertilized-2 blasts transferred on day 5-BFN, FET transfer where 2 were transferred-BFN, final FET transfer 3 transfer-HCG level was 4 and a chemical pregnancy was diagnosed.

Current donor egg cycle- 2 blastocysts transferred on 4/17-on 4/26 I received a low positive of 16.1 and since then hcg has gone up as follows...

I was told I would be considered 6 weeks pregnant but at the us on 5/11 there was nothing seen and I was advised to stop all meds and that this was most likely chemical or an ectopic. I return on 5/14 for additional bloodwork. I do have some cramping and am bleeding like a regular period.

I have 3 frozen embryos but don't want to keep doing the same thing, do you think there might be an immune issue here? This time it seemed like the HCG is still going up appropriately but nothing seen, would that happen with an ectopic?

I would like to make a consultation but wanted to see if you agreed there might be something missing from my treatment or have I just had bad luck?

Hi Mary , no this is not bad luck. Failed donor egg cycles are almost always an immune issue, I believe in your case related to the endometriosis. This can be treated and the majority of our patients that present with your history leave with a baby. I agree, I would not transfer the remaining embryos until we figure out your problem . It turned out to be ectopic. HCG reached nearly 5000 and the ectopic was located in my left tube. i had surgery and the tube was removed.

At the time of transfer I did have the intrauterine hcg injection, could this have caused excess fluid that allowed the embryo to float to the tube?

Does the fact it turned out to be ectopic have any impact on whether or not I may have an immune issue?

Frustrated because it implanted just in the wrong place!
