Intralipid Risks

Dear Dr Braverman,


I have a failed ICSI and failed FET cycle and are about to embark on our second fresh ICSI cycle. We are being treated for male factor issues but my NK cells have come back high (25.2) as well as my TNFa (38.1). I don't feel comfortable taking humira so my consultant has recommended intralipds.


I am concerned about any potential risks of intralipids also. I have a fibroadenoma on my breast and although it has been confirmed as benign, I am worried that having intralipids may cause the benign tumour to grow/multiply or to increase my risk of malignancy and cancer. I am wondering whether, if tumours can be affected by a diet high in fat, whether intralipids may be detrimental. I understand that you may not be able to comment on the fibroadenoma, but are you aware of any potential links to intralipids and cancer risks at all or any other health risks?


I want to give myself the best possible chance of pregnancy without risking my health. I am therefore wondering if steroids alone may be helpful for lowering my TNFa or whether by looking at my levels, it is likely that the use of intralipids may be necessary.

As I so often state in this forum, the very limited testing you have had does not help you decide on therapy. You should not be basing your treatments or the need for prednisone on limited testing. I do feel however that limited intralipids are not of concern to you.

I was recommended hurmira ( plus additional medications: steroids, neupogen, clexane) but wanted to choose intralipids as they appear more benign option and have been advised to take 2 lots of intralipids 3 weeks apart to see how my body responds. Would you recomend a different protocol/additional tests based on my limited results?


Also, you said you are not concerned about the intralipids- just wantd to clarify that you are referring to the lack of risk around intralipids and my fibroadenoma?

Thank you. Yes, I understand that it is still very limited and will certainly look at your website in more detail. We are undecided about who to seek treatment from so a second opinion/consultation would certainly be useful.


In terms of my fibroadenoma- if intralipids were a relevant treatment option following further testing, would intraipids pose a risk for my benign tumour or have you treated patients with history of tumour/cancer previously without concerns?

you must ultimately follow the advice of your doctor , but I would not be concerned if a patient of mine had fibroadenoma and used intralipids at the usual doses that we use .
