IVF with Crohn's disease

Hi Dr. Braverman,
I am 33yo and am headed into my 4th IVF attempt. Medically I have a 20yr h/o Crohn's disease and also have fairly mild psoriasis and occasional alopecia areata. From an infertility standpoint, I have blocked tubes and high FSH/DOR (both from adhesions s/t 10-15 major abdominal surgeries for Crohn's). In my 3 cycles, I have had 2 transfers- the first with a 5-cell and a 3-cell and the second with an 8-cell and a 5-cell. With this last cycle, it appears an embryo briefly implanted (given pos HPTs 8 and 9dp3dt and very high prog and E2 levels) but beta was only 0.8 at 15dpo.

My question is- While I have not had repeated losses, I have a couple autoimmune conditions which make me wonder if I should have further immune testing. My ANA has been negative every time it's been checked (several times over the years) and antiphospholipid antibody panel was well wnl. Would you suggest further testing? If so, what would you suggest?

it seems clear that you have 2 issues , one is your DOR ? secondary to the surgeries or even it may be autoimmune, but there is also great potential for implantation issues due to the high levels of TNF associated with Crohns. It would be best to cal and schedule a consultation so I can review your hitory and get you the proper testing. At that point I can advise on the best plan of management for you.
