Letrozole - explanation - with a flare?

Hello Dr Braverman,

I was wondering how/if letrozole can be used with a flare protocol? Do I understand correctly that the dosage of stimmulation drug would have to be reduced - how is the reduction calculated?

The reason I ask is that I have endometriosis and low AMH.
I have a phone consultation with you soon and I am trying to see if my doctor here in the UK could with alondgside your consultations for my next cycle.

I willl be discussing the idea of adding letrozole with my doctor BEFORE I get the chance to discuss it with you. I realise you can't give advice about my individual case - I am just asking generally in regards to how it could help someone LIKE me.

My doctor here put his hands up and said he had no experience using letrozole but was still very interested - his initial thought was that it might have a similar affect as the contraceptive pill (which I'm on before next cycle) - but I understood it to be very different in its affects - but I don't know how to explain this and I want to be able to put forward the case - if it is indeed possible to use it with a flare.

Any explanation you can give would be very much appreciated.

letrazole works much the same way as lupron does in the flare protocol. it causes the release of LH and FSH that assists the gonadotropin injections that you will be using. I generally go wtih 375 to 450 follistim for those over 40.

It is the release of FSH and LH that allows us to use letrazole as a fertility drug for follicle development in IUI cycles as well. For endometriosis it deactivates the aramotase enzyme to assist with implantation.
