neupogen and high WBCs/ANC

Wondering why some patients exhibit extremely elevated WBCs (50K and up) and ANCs when on neupogen while other patients are able to stay within a range of 20-30K WBCs the course of the whole trreatment? Why is this? ( other than patients respond differently to different drugs).

1. Are patients with elevated WBCs responding or not responding well to neupogen?

2. with high numbers but no other s/s do you keep them at their same dose even if WBC goes over 50K? (I understand you also look at other factors as well when determining this.)

There are many reasons why patients respond differently and these answers are not obvious . some have more receptors of the GSCF, some have different rates of clearance just to name a few. You are correct , I look more at the immune changes than the WBC determine the effectiveness of treatment.
