Hello,


I have been found to be missing the three KIR receptors. Neupogen has been recommended however, due to potential risks/side effects I do not feel 100 % sure about taking this medication. I am a monozygotic twin and my sister has had two uneventful natural pregnancies. I am unsure as to whether there is reported concordance of KIR receptors amongst identical twins.


I am receiving intralipids, prednisolone and clexane for raised tnfalpha (reduced by intrlaipids to 26 at start of cycle.) I am also using embryogen, which whilst I understand that this works differently from GCSF I am wondering if that may be helpful for my missing KIRs and uterine receptivity.


As I do not wish to pursue the neupogen injections at this time, the neupogen wash has been recommended to me on its own. I have been advised to have it the day after egg collection ahead of what will hopefully be a 5 day transfer. I do not have any issues with my womb lining (currently 11.5)


My questions is therefore- in your professional opinion would it be potentially helpful to have the wash to improve uterine receptivity if I am missing the three KIRS?


Is there a risk of allergic reaction/inflammation in the womb or would this potentially be detrimental if I am also using the culture medium GM CSF?

As you and your sister have already proven , KIR issues are rarely the cause of implantation issues. Also Intralipids do not treat "TNF" issues in fact the ratios used to even determine if you have raised TNF are not valid. As with most patients that contact me , you have not had the correct testing to make a diagnosis. The treatmetns offered at this point are merely guess work. Thank you for your reply. That is reassuring that the KIR receptors are not solely responsible for implantation failure.


We are currently committed to our ICSI cycle, hence why we did not contact you for a conuslt earlier although we would certainly be keen to explore cycling with your clinic, for any future treatment and would love to arrange a free consult.


Are you able to clarify whether the neupogen wash is mainly indicated to improve uterine thickness or receptivity and can it be worth doing without the follow up injections in order to improve implantation rates?

only for lining issues, no evidence for any other benefit. We can consult and collaborate with your clinic you dont need to come here at all. I recommend you dont proceed with any transfers until you have a correct diagnosis.

Thank you for clarifying that. I do not have any lining issues whatsoever so I guess it is unlikely I would beneft from this wash.
