Hi Dr. Braverman!

My husband and I are 23 and 24 years old and have been trying for almost 2 years. During those 2 years we got pregnant once on accident (on BCPs), but it ended in chemical pregnancy. After that, nothing. We went and got testing done. I was originally told I had low progesterone and my husband had low motility and morphology, but those were later retracted after more sperm tests and 4 rounds of IUI with great response and progesterone supplements. We were also told my husband had high DNA fragmentation in his sperm, so we did IVF with assisted hatching and ICSI.

Our first IVF cycle we had 28 retrieved, 14 mature, 11 fertilized. We transferred 2 perfect blastocysts on day 5 and my first beta was 41, 2 days later it had dropped to 40. So yet another chemical pregnancy. We have one frozen embryo.

Along the way I also did acupuncture, tried dexamethasone, baby asprin, Metfromin, and went to an endocrinologist because of a family history of thyroid problems, but he found nothing except some benign cysts.

I actually believe that the IUIs resulted in fertilization a few times, but that they could never "stick". I felt that this also happened before naturally, because I did on one other occasion get a positive test, and then the next day or 2 would start a period. I could never specifically confirm this though, as blood tests would be negative.

I convinced my RE to run a recurrent loss panel and she found I had elevated lipoprotein a and my insulin was higher then she would like. So I am back on a form of metformin and she would like to try lovenox, but she believes we should just press forward and assume the problem was with the embryo. I have my doubts as I get cold like symptoms after each IUI and the day I found out my beta had dropped. This has also happened on natural cycles. I just have a feeling ferlitization isn't our issue and for whatever reason, they aren't "sticking" once they get there.

Yes I agree, it is quite possible that this is an implantation issue. These disorders are quite complex and you should have a correct workup(I know I say that alot on this forum but most workups for this issue are significantly indaquate.
