Risks of taking Prednisolone

Dear Dr Braverman,

May I please have your advice on the following questions:

  • The longer Prednisolone is taken, the greater the risks of bone thinning because steroids impair calcium absorption. Would you recommend your patients to take extra calcium supplement? If so, at what dose?
  • For the unborn baby, there is an increased risk of birth defects such as cleft lip and cleft palate. Would you recommend taking extra folic acid with the aim of reducing such risk. If so, at what dose do you recommend?

our patients take an extra 1200mg of Calcium per day. And we rarely see cleft palate with our prednisone patients and there are no studies to show that folic acid supplementation in this setting will reduce the risk any further, however taking an extra 2mg per day would not hurt.
Again I am not familiar with your case and final decisions on management must be with your doctor.
