RPL and ATA/ANA antibodies

Hi Dr. Braverman,

My primary question is: is presence of low titer ANA and ATA considered strictly an immune issue, or is there also a link to blood clotting?

I am 36 and in the past 2 years I have experienced 3 miscarriages. I had a healthy son 4 years ago but that pregnancy was complicated by bleeding in the 3rd trim believed to be due to a small placental clot.

I was on no prescription meds for my healthy preg or first two m/c. For the second m/c, beta hCG and prog looked great but we never got to the point of first u/s because I started bleeding. After 2nd loss, I underwent RPL testing and tested positive for Anti-thyroglobulin (approx 140) and low titer ANA (1:40). Everything else including HSG were normal. ANA was evaluated further and nothing came up. Other thryoid tests were normal. Homocysteine levels normal but MTHFR not tested (is this the norm?)

I started low dose synthroid and baby aspirin and I conceived again. My OB wondered if lovenox might help but MFM and rheum said no because I have no documented clotting disorder. We saw HB at 7.5 weeks (>150 bpm)- but we learned at 10w it was over- baby stopped growing around 8 weeks. Had d&c and fetal karyotype was normal. We are heartbroken after 3 consecutive losses.

I should also mention that I have mild psoriasis and raynauds. I have always had very short, very light periods but have been reasurred that this is not related because I am ovulating and getting pregnant naturally. Also, I had painful CL cysts in both my healthy pregnancy and this last m/c.

With the positive ANA, I would recommend Lovenox treatment. Please call my office to schedule a consult and I will review my recommendations in detail.
