Ideal WBC levels when on Neopogen?

Hi Dr. B,


I hope all is well. I am currently taking Neopogen and my WBC was 49.9 and I am in the two week wait. Currently on CD5 on a 3 day transfer. I am taking the standard dose fof Neopogen for my weight. In Reproductive Immunology what is the ideal level during the TWW that you prefer when patients are on Neopogen that have a history of high NK activity? Thank you,

First you have to understand the WBC is only one small parameter to look at when taking neupogen. In fact there is no proven associaton between the WBC and the effect. More important to follow T regulator cell migration and adjust the dose based on this. NK activity is of no value in determining the need for "any" immune therapy. So it is hard to answer your qeustion. I will tell you that 49.9 is not an unusual level in those on neupogen and in my patients (where I know thier complete history) this would not alarm me.
